  2. Schedule Your Screenings:<\/strong> Choose an eligible health screening and make an appointment with your provider to complete them.<\/li>\n\n\n\n
  3. Complete Your Screenings:<\/strong> Visit your provider to complete screening.<\/li>\n\n\n\n
  4. Send Your Card In:<\/strong> Follow the instructions on the card for validation.\n
